ABSTRACT

Introducción: En la actualidad se emplean nuevas alternativas no invasivas para la remoción de diversos procesos inflamatorios del epitelio mucoso bucal, siendo la crioterapia una técnica fisioterapéutica basada en la aplicación del frío intenso con nitrógeno líquido para producir en la piel o mucosas del organismo una congelación y destrucción local de células y tejidos afectados de forma controlada. Objetivo: Evaluar los efectos terapéuticos de la criocirugía en pacientes geriátricos portadores de estomatitis subprótesis. Métodos: Se efectuó un estudio descriptivo y transversal de 50 pacientes adultos mayores, de ambos sexos, diagnosticados con estomatitis subprótesis y asistidos en la consulta estomatológica del Policlínico de Especialidades referente al Hospital Clinicoquirúrgico Universitario Saturnino Lora Torres de la provincia Santiago de Cuba, en el periodo comprendido desde marzo de 2022 hasta igual mes de 2023. Resultados: Se evidenció que las congelaciones entre 20 y 30 segundos y los deshielos entre 91 y 120 segundos fueron los más eficaces en la destrucción de la mucosa palatina dañada, logrando la regeneración tisular en un tiempo mínimo con la indicación de uno a tres ciclos de enfriamientos rápidos y deshielos lentos en una sesión terapéutica. La ausencia de síntomas fue lo más relevante en la evolución postquirúrgica. Conclusiones: La crioterapia demostró ser un método eficaz e inocuo para regenerar los tejidos lesionados al realizar congelaciones rápidas con deshielos lentos. Este método crioquirúrgico, fácil de aplicar y atraumático, logró una óptima reparación y reepitelización de los tejidos bucales dañados con excelentes respuestas terapéuticas, evitando complicaciones postquirúrgicas como la infección y el sangramiento local.


Introduction: At present, new non-invasive alternatives are used for the removal of various inflammatory processes of the oral mucous epithelium; being cryotherapy a physiotherapeutic technique based on the application of intense cold with liquid nitrogen to produce freezing on the skin or mucous membranes of the organism and local destruction of affected cells and tissues in controlled manner. Objective: To evaluate the therapeutic effects of cryotherapy in geriatric patients with sub-prosthesis stomatitis. Methods: A descriptive and cross-sectional study of 50 older adult patients, of both sexes, clinically diagnosed with subprosthesis stomatitis and assisted in the dentistry consultation of the Specialties Polyclinic referring to the Saturnino Lora Torres University Clinical Surgical Hospital in the city of Santiago de Cuba, in the period from March 2022 to the same month of 2023. Results: It was evidenced that freezing between 20 and 30 seconds and thawing between 91 and 120 seconds were the most effective in the destruction of the damaged palatal mucosa, achieving tissue regeneration in a minimum time with the performance of 1 to 3 cycles of rapid cooling and slow thawing in a therapeutic session. The absence of symptoms was the most relevant in the post-surgical evolution. Conclusions: Cryotherapy proved to be an effective and innocuous method to regenerate injured tissues when fast freezing with slow thawing. This cryosurgical method, easy to apply and atraumatic, achieved optimal repair and re-epithelization of damaged oral tissues with excellent therapeutic responses, avoiding post-surgical complications such as infection and local bleeding.

ABSTRACT

O controle de formação de biofilme deve ser promovido por meio de técnicas diárias de higienização, sendo uma delas o uso de soluções de desinfecção. Dentre as soluções químicas comercialmente disponíveis, algumas podem ocasionar efeitos adversos quando utilizadas em longo prazo. Dessa forma, a busca por métodos alternativos de desinfecção, com soluções que não alterem as propriedades do material e que sejam inertes para o seu usuário torna-se essencial. Este estudo teve como objetivos: avaliar in vitro, a ação antifúngica de formulações de 5 mg/mL à base do óleo de citronela em biofilmes mistos de espécie de Candida em resina acrílica ativada termicamente (RAAT) para base protética por meio de contagem do número de unidades formadoras de colônias (UFC/mL), em dois tempos de exposição (15 e 30 minutos) e avaliar a citotoxicidade das formulações em células da linhagem epitelial HaCat, e verificar a ação destas formulações de citronela a 5 mg/mL na alteração de propriedades físicas e mecânicas de amostras de RAAT. Inicialmente, foram feitas as formulações à base de citronela em forma de emulsão e nanoemulsão. Em seguida, por meio de ensaios de microdiluição em caldo, foi obtida a concentração inibitória mínima (CIM) e concentração fungicida mínima (CFM) das cepas de Candida testadas. Foram confeccionadas amostras de RAAT para a realização dos ensaios, sendo 96 amostras para o ensaio de biofilme, divididas de acordo com os grupos: GI - controle negativo (Biofilme misto C. albicans ATCC + C. glabrata ATCC + C. albicans oral 6892 + C. albicans oral 7037); GII- Nanoemulsão de Citronela 5mg/mL; GIIIControle da Nanoemulsão (Tween 20); GIV- Emulsão de Citronela 5mg/mL; GV- Controle da Emulsão (Goma Xantana); GVI- Clorexidina 0,12% (Periogard), e 20 para o ensaio de viabilidade celular, divididas nos grupos: GI- Nanoemulsão de Citronela 5mg/mL; GII- Controle da Nanoemulsão (Tween 20); GIII- Emulsão de Citronela 5mg/mL; GIV- Controle da Emulsão (Goma Xantana); GV- Clorexidina 0,12% (Periogard). Biofilmes mistos de Candida foram formados sobre as superfícies das amostras, e foram quantificados por meio de contagem de unidades formadoras de colônias (UFCs). Para o teste de viabilidade celular utilizou-se células epiteliais da linhagem HaCaT, avaliadas através da coloração com resazurina. Para avaliação das propriedades físicas e mecânicas foram confeccionadas 50 amostras retangulares e 50 circulares de RAAT, separadas aleatoriamente em 5 grupos, de acordo com a formulação utilizada: GI ­ controle (água destilada); GII ­ saliva artificial; GIII ­ nanoemulsão de citronela a 5mg/mL; GIV - emulsão de citronela a 5mg/mL e GV - Clorexidina 0,12% (Periogard). As amostras foram imersas nas respectivas formulações, simulando a desinfecção de próteses dentarias pelo período de 30 minutos diários, durante 6 meses. Após este período, foi feita a análise da alteração de rugosidade de superfície (Ra - µm), estabilidade de cor (CIEDE 2000), avaliação da microdureza Knoop e resistência a flexão (MPa) das amostras. Os dados obtidos foram submetidos às análises estatísticas (p< 0,05). As formulações fitoterápicas à base de citronela apresentaram valores de CIM e CFM que variaram entre 0,156-1 mg/mL sobre as espécies de Candida e inibiram o crescimento do biofilme misto, apresentando uma redução estatisticamente significativa na contagem de UFC/mL, de até 2,7 Log destes fungos, principalmente quando submetidas ao tempo de exposição de 30 minutos. As formulações fitoterápicas testadas não apresentaram diferenças estatísticas entre si, apresentando viabilidade da célula HaCaT maior que 70%, diferentemente da clorexidina a 0,12%. Não houve diferença nos valores médios de rugosidade de superfície em nenhum dos grupos após serem submetidos à imersão nas formulações testes, com exceção do grupo controle, que apresentou diferença estatística significativa na rugosidade de superfície após o período de imersão. Nos valores de alteração de cor (ΔE00) foi possível observar que não houve diferença estatística significativa entre os grupos experimentais antes e após serem submetidos a imersão nas formulações testes. Nos valores de microdureza Knoop houve diferença estatística significativa apenas entre o grupo controle e o grupo emulsão de citronela. Para os dados de resistência flexural, módulo de elasticidade, força máxima e alongamento não houve diferença estatística significativa entre os grupos após imersão. Em suma, concluiu-se que as formulações fitoterápicas à base de citronela apresentaram efeito antifúngico sobre as diferentes espécies de Candida em superfície de resina acrílica para próteses dentárias, não apresentaram efeito citotóxico sobre células da linhagem epitelial, e se mostraram seguras como formulações antissépticas no que se refere às propriedades físicas e mecânicas estudadas, sendo potencialmente promissoras para serem indicadas como soluções desinfetantes para próteses dentárias(AU)


The control of biofilm formation should be promoted through daily hygiene techniques such as the use of disinfection solutions. Among the commercially available chemical solutions, some can lead to adverse effects in long-term use. Thus, the search for alternative methods of disinfection with solutions that do not interfere with properties of the material and those inert to the user becomes essential. This study aimed to: evaluate, in vitro, the antifungal effect of 5 mg/mL formulations based on citronella oil on mixed biofilms of Candida species in thermally activated acrylic resin (TAAR) for prosthetic base by counting the number of colony forming units (CFU/mL), in two exposure times (15 and 30 minutes) and to evaluate the cytotoxicity of the formulations in cells of the HaCat epithelial lineage, and to verify the action of these formulations of citronella at 5 mg/mL in the alteration of physical and mechanical properties of TAAR samples. Initially, citronella-based formulations were prepared as emulsion and nanoemulsion. Then, by means of broth microdilution assays, the minimum inhibitory concentration (MIC) and minimum fungicidal concentration (MFC) of the tested Candida strains were obtained. For the biofilm test, 96 TAAR samples were prepared and assigned to the groups, as follows: GI - negative control (Mixed biofilm C. albicans ATCC + C. glabrata ATCC + C. albicans oral 6892 + C. albicans oral 7037); GII- Citronella Nanoemulsion 5mg/mL; GIII- Nanoemulsion Control (Tween 20); GIV- Citronella Emulsion 5mg/mL; GVEmulsion Control (Xanthan Gum); GVI- Chlorhexidine 0,12% (Periogard). For the cell viability assay, 20 samples were prepared and assigned to the following groups: GI- Citronella Nanoemulsion 5mg/mL; GII- Nanoemulsion Control (Tween 20); GIII- Citronella Emulsion 5mg/mL; GIV- Emulsion Control (Xanthan Gum); GV- Chlorhexidine 0,12% (Periogard). Mixed Candida biofilms were formed on the surfaces of the samples, and were quantified by counting colony forming units (CFUs). For the cell viability test, epithelial cells of the HaCaT lineage were used, evaluated by staining with resazurin. To evaluate the physical and mechanical properties, 50 rectangular and 50 circular samples of TAAR were prepared and randomly assigned into 5 groups, according to the formulation used: GI ­ control (distilled water); GII ­ artificial saliva; GIII ­ citronella nanoemulsion at 5mg/mL; GIV ­ citronella emulsion at 5mg/mL and GV ­ chlorhexidine 0,12% (Periogard). The samples were immersed in the respective formulations, simulating the disinfection of dental prostheses for a period of 30 minutes daily, for 6 months. After this period, the analysis of surface roughness change (Ra - µm), color stability (CIEDE 2000), evaluation of Knoop microhardness and flexural strength (MPa) of the samples was performed. The data obtained were submitted to statistical analysis (p< 0,05). The phytotherapic formulations based on citronella presented MIC and MFC values that varied between 0,156-1 mg/mL on Candida species and inhibited the growth of mixed biofilm, showing a statistically significant reduction in the CFU/mL counts, up to 2,7 Log of these fungi, mainly when subjected to an exposure time of 30 minutes. The phytotherapic formulations tested did not show statistical differences between them, showing HaCaT cell viability greater than 70%, unlike 0,12% chlorhexidine. There was no difference in the mean values of surface roughness in any of the groups after being subjected to immersion in the test formulations, except for the control group, that presented a statistically significant difference in surface roughness after the immersion period. Regarding color change (ΔE00) it was possible to observe that there was no statistically significant difference between the experimental groups before and after being subjected to immersion in the test formulations. In the Knoop microhardness values, there was a statistically significant difference only between the control group and the citronella emulsion group. For data on flexural strength, modulus of elasticity, maximum force and stretching, there was no statistically significant difference between the groups after immersion. In summary, it can be concluded that phytotherapic formulations based on citronella had an antifungal effect on different Candida species on acrylic resin surfaces for dental prostheses, besides not presenting a cytotoxic effect on cells of the epithelial lineage, and proved to be safe as antiseptic formulations in the which refers to the physical and mechanical properties studied, being potentially promising to be indicated as disinfectant solutions for dental prostheses(AU)

Aim: To evaluate the effect of three natural antifungal agents combined with routine denture care on the treatment of DS, using a quantitative mycological culture analysis. Methods: Thirty denture wearers with denture stomatitis DS were treated using five substances: sterile distilled water (G1), nystatin oral suspension (G2), 20% alcoholic extract propolis (G3), Punica granatumLinné gel (G4), and Uncaria tomentosa gel (G5). The substances were used 3 times a day for 14 days. Quantitative mycological culture analysis of samples collected from the palatal mucosa was performed at three stages: before treatment (T0), after 14 days of treatment (T1), and 30 days after treatment completion (T2). Data were evaluated using Kruskal-Wallis and Friedman tests (p < 0.05). Results: Palatal mucosa intragroup analysis showed a significant reduction of Candida CFU/mL values for all groups at T1 compared to T0 (p < 0.05). However, they did not present statistical differences when comparing T1 and T2 (p > 0.05). The intergroup analysis demonstrated that there are no statistical differences, regardless of the evaluation time (p > 0.05). Conclusion:The natural products tested showed a satisfactory result on DS treatment, which proved to be equivalent to conventional topical therapy with nystatin and to treatment using only regular oral hygiene procedures.


Objetivo: Avaliar o efeito de três antifúngicos naturais combinados com o cuidado rotineiro com próteses dentárias no tratamento da EP, por meio de uma análise quantitativa de cultura micológica. Métodos: Trinta usuários de próteses dentárias com EP foram tratados com cinco substâncias: água destilada estéril (G1), suspensão oral de nistatina (G2), extrato alcoólico de própolis 20% (G3), gel Punica granatum L. (G4) e gel Uncaria tomentosa (G5). As substâncias foram utilizadas 3 vezes ao dia durante 14 dias. A análise micológica quantitativa das amostras coletadas da mucosa palatina foi realizada em três etapas: antes do tratamento (T0), após 14 dias do tratamento (T1) e 30 dias após o término do tratamento (T2). Os dados foram avaliados pelos testes de Kruskal-Wallis e Friedman (p < 0,05). Resultados: A análise intragrupo da mucosa palatina mostrou uma redução significativa dos valores de Candida UFC/mL para todos os grupos em T1 em comparação com T0 (p < 0,05). No entanto, não apresentaram diferenças estatísticas na comparação de T1 e T2 (p > 0,05). A análise intergrupos demonstrou que não há diferenças estatísticas, independentemente do tempo de avaliação (p > 0,05). Conclusão: Os produtos naturais testados apresentaram resultado satisfatório no tratamento da EP, sendo equivalente à terapia tópica convencional com nistatina e ao tratamento apenas com procedimentos rotineiros de higiene bucal.

The ageing of population is increasing, and a great percentage of these patients wear removable prostheses, and can suffer denture stomatitis, a condition that has been associated with candidiasis. Aims: To evaluate in vitro the effectiveness of Copper Sulfate against Candida albicans in samples of heat-polymerized acrylic resin, compared to nystatin, sodium hypochlorite and chlorhexidine. Materials and Methods: Initially, the minimum inhibitory concentration (MIC) of copper sulfate for Candida albicans was determined by microdilution. Then, 54 resin samples were divided into 6 treatment groups corresponding to Nystatin 100.000 UI, Sodium Hypochlorite 0.5%, chlorhexidine 0.12%, Copper Sulfate 4.7µg/ml, Copper Sulfate 9.4µg/ml and physiological saline solution, in which samples were submerged for 6 hours. Resin samples were then washed and cultured on solid media at 37°C for 72 hours. The number of colony-forming units was determined using a Quebec colony counter. The statistical analysis was performed using the Kruskal-Wallis test and the Mann-Whitney U test. Results: Copper sulfate at a concentration of 9.4µg/ml presented a similar effectiveness as the other control products regarding the reduction in the number of colonies of Candida albicans post-treatment. Conclusion: The effectiveness of copper sulfate against Candida albicans on acrylic resin was similar to that of nystatin, sodium hypochlorite and chlorhexidine.


En las últimas décadas se ha observado un aumento de la población de adultos mayores, de los cuales un gran porcentaje es portador de prótesis removible, y dos tercios pueden sufrir estomatitis subprotésica, enfermedad que es asociada a infecciones como candidiasis. Objetivo: Evaluar la efectividad antimicótica in vitro del sulfato de cobre en placas de resinas acrílicas de termocurado inoculadas con Candida albicans, frente a Nistatina, Hipoclorito de Sodio y Clorhexidina. Material y Métodos: Inicialmente, y mediante microdilución del sulfato de cobre, se determinó la concentración mínima inhibitoria (CMI) para Candida albicans. En la fase experimental, 54 muestras de resina se dividieron en 6 grupos correspondientes a Nistatina 100.000 UI, Hipoclorito 0.5%, Clorhexidina 0.12%, Sulfato de Cu 4.7µg/ml, Sufato de Cu 9.4 µg/ml y suero fisiológico. Las muestras fueron sumergidas en estos agentes por 6 horas, para posteriormente ser lavadas y cultivada en medios solidos a 37°C por 72 horas. Luego se realizó el conteo de unidades formadoras de colonias mediante contador tipo Quebec. El análisis estadístico se realizó mediante la prueba de Kruskal-Wallis y la prueba U de Mann-Whitney. Resultado: El sulfato de cobre a una concentración de 9.4µg/ ml presentó una efectividad similar a los otros productos, en la reducción de colonias de Candida albicans. Conclusión: La efectividad del sulfato de cobre contra Candida albicans fue semejante a la de Nistatina, Hipoclorito y Clorhexidina.

Abstract Objective: To evaluate the surface morphology and in vitro leachability of temporary soft linings modified by the incorporation of antifungals in minimum inhibitory concentrations (MIC) for Candida albicans biofilm. Methodology:Specimens of soft lining materials Softone and Trusoft were made without (control) or with the addition of nystatin (Ny), miconazole (Mc), ketoconazole (Ke), chlorhexidine diacetate (Chx), or itraconazole (It) at their MIC for C. albicans biofilm. The surface analyses were performed using Confocal laser scanning microscopy after 24 h, 7 days, or 14 days of immersion in distilled water at 37ºC. In vitro leachability of Chx or Ny from the modified materials was also measured using Ultraviolet visible spectroscopy for up to 14 days of immersion in distilled water at 37ºC. Data (µg/mL) were submitted to ANOVA 1-factor/Bonferroni (α=0.05). Results: Softone had a more irregular surface than Trusoft. Morphological changes were noted in both materials with increasing immersion time, particularly, in those containing drugs. Groups containing Chx and It presented extremely porous and irregular surfaces. Both materials had biexponential release kinetics. Softone leached a higher concentration of the antifungals than Trusoft (p=0.004), and chlorhexidine was released at a higher concentration than nystatin (p<0.001). Conclusions: The surface of the soft lining materials changed more significantly with the addition of Chx or It. Softone released a higher concentration of drugs than Trusoft did, guiding the future treatment of denture stomatitis.

Abstract Here, the prevalence of oral candidiasis and denture stomatitis among diabetic patients compared to healthy ones was summarized through a systematic review with meta-analysis. Medline, Scopus, Web of Science, Lilacs, Cochrane Library, Embase, and the grey literature were searched without restriction, until May 2020. Eligibility criteria were established, data were extracted, and quality assessment was conducted by two trained examiners. Qualitative synthesis was based on the recommendations of Fowkes and Fulton. Two meta-analyses were performed on studies investigating patients with: a) oral candidiasis and b) denture stomatitis. Out of 6034 screened studies, seven were eligible for qualitative and quantitative synthesis; of these, three evaluated oral candidiasis and four evaluated denture stomatitis. Qualitative synthesis showed that the main methodological problems of the studies included sample size, source of controls, matching, and randomization. Diabetic patients had a similar chance of developing oral candidiasis to non-diabetic patients (OR1.40 [0.96; 2.04], p = 0.08, I2 = 94%). However, diabetic patients had a higher chance to present denture stomatitis compared to non-diabetic patients (OR 1.92 [1.42, 2.59] p < 0.0001, I2 = 0%). Therefore, diabetic patients have a higher chance of developing denture stomatitis compared to non-diabetic patients. However, for all analyses, the certainty of the evidence was considered to be very low.

ABSTRACT

Dentures with dental plaque predispose recurrent hyperplasia on the palatal mucosa. Surgical procedures for the treatment of inflammatory papillary hyperplasia involve postsurgical discomfort and morbidity. This repot describes clinical and histologic aspects of a patient with severe akantolitic inflammatory papillary hyperplasia. The palatal mucosa was treated with a surgical bur with a low-speed handpiece. A new removable denture was performed and adapted. A follow-up of 4 years showed staility of health at palatal mucosa. Patient referred low discomfort and morbidity when using bur technique. Control of removable denture was critical for long-term healing and soft tissue stability.


Las prótesis dentales con placa bacteriana predisponen a la hiperplasia recurrente en la mucosa palatina. Los procedimientos quirúrgicos para el tratamiento de la hiperplasia papilar inflamatoria implican molestias y morbilidad posquirúrgicas. Este reporte describe los aspectos clínicos e histológicos de un paciente con hiperplasia papilar inflamatoria acantolítica severa. La mucosa palatina fue tratada con una fresa quirúrgica con una pieza de mano de baja velocidad. Se realizó y adaptó una nueva prótesis parcial removible. Después de un seguimiento de 4 años, se encontró estabilidad de la salud de la mucosa palatina. El paciente se refirió a la baja incomodidad y morbilidad al usar la técnica de la fresa. El control de la prótesis parcial removible fue crítico para la cicatrización a largo plazo y la estabilidad de los tejidos blandos, evitando la recidiva de la hiperplasia papilar inflamatoria.

ABSTRACT

Aim: The prevalence of denture stomatitis and the relationship with its risk factors was evaluated via secondary data from 62 complete denture (CD) wearers, between 2015 and 2017. Methods: The data was stored in an Excel database and was analyzed using the statistics software STATA/SE 12.0. A descriptive analysis was performed taking into account a categorization of the clinical variables according to risk factor in 4 domains: CD usage, systemic health, hygiene habits, and usage habits. The association amongst the denture stomatitis and risk factors variables was tested by the Chi-square test and the results were statistically significant at p-values <0.05. Results: The CD wearers participants were composed by a majority of elderly (80.64%) and women (72.78%); with 45.16% having been using the current denture for more than 10 years and another 74.19% related a continuous usage. Diabetes and hypertension were related by 83.87% and 67.74%, respectively, with 87.10% using medication. Although 75.80% reported cleaning their dentures 3 times per day or more, and 59.68% considered their oral hygiene very good, 50% of the complete dentures showed dirtiness and clinical signs of Denture Stomatitis were present in 30.64% of the patients. Despite of the evaluated sample shows many risk factors for the development of the disease, an association between the variables was not observed. Conclusion: Considering the high prevalence of the disease in the studied sample, preventive and educational measures on denture usage and hygiene must be reinforced in order to maintain the oral health of the edentulous patients

ABSTRACT

O objetivo deste estudo piloto foi comparar a eficácia de um reembasador acrílico macio com ou sem diacetato de clorexidina a 1% no tratamento de pacientes com estomatite protética. As hipóteses testadas foram: (I) o eritema seria melhorado (IIa): a contagem de unidades formadoras de colônias seria reduzida, (IIb): a predominância de um tipo morfológico de Candida seria correlacionada com características clínicas e, (IIc) se uma concentração inibitória mínima do antimicrobiano, poderia ser capaz de atuar sobre o crescimento do fungo. Dezesseis pacientes desdentados totais, portadores de prótese e com estomatite protética foram distribuídos aleatoriamente para os grupos controle (reembasamento com Trusoft) e grupo teste (reembasamento com Trusoft contendo 1% de clorexidina) sendo acompanhados por exames clínicos e laboratoriais por 14 dias. Os parâmetros analisados nas duas consultas de acompanhamento foram: avaliação clínica (exame e fotos) e avaliações microbiológicas (esfregaços e culturas de palato e prótese total). Este estudo foi um ensaio clínico randomizado, duplo-cego,controlado.Os resultados obtidos demonstraram que a superfície do eritema foi significativamente reduzida, mas não diferiu significativamente entre os dois grupos (P> 0,05), assim como a redução da contagem de Candida durante o tratamento entre os grupos (P> 0,05). O tipo de Candida não pôde ser correlacionado ao estágio clínico alcançado. Os resultados dos testes de concentração inibitória mínima, revelaram que uma concentração de 0,05 µg/ml de clorexidina já é capaz de inibir o crescimento de 85% das cepas de testadas.A hipótese principal foi confirmada, o que pode ser interpretado como uma remissão clínica acelerada em ambas as terapias propostas. Embora as hipóteses secundárias não tenham sido confirmadas, a persistência de Candida pode ser apenas a colonização, e a plasticidade morfológica de Candida parece ser fortemente influenciada pela virulência fúngica. Sendo assim, os resultados da análise de concentração inibitória mínima sugerem que uma concentração subinibitória de clorexidina possa estar atuando na virulência da Candida spp. No entanto, não foram encontrados resultados para confirmar a eficácia clínica do diacetato de clorexidina a 1% incorporado ao reembasador macio testado no tratamento da estomatite protética.


The aim of this pilot study was to compare the efficacy of rebase using soft acrylic liner with or without 1% chlorhexidine diacetate in denture stomatitis patients. It was hypothesised that: (I) erythema would be improved (IIa): colony forming unit count would be reduced, (IIb): the predominance of a morphological type of Candida would be correlated with clinical features and, (IIc) a minimum inhibitory concentration of the antimicrobial, could be able to act on the growth of the fungus.Sixteen patients who wearing removable dentures and diagnosed with denture stomatitis were randomly assigned to the control groups (rebase with resilient liner) and test group (rebase with resilient liner containing 1% of chlorhexidine), followed by clinical and laboratory tests for 14 days. The analyzed parameters at two follow-up were: clinical evaluation (examination and photos) and microbiological evaluations (smears and cultures of palates and dentures). This study was a randomized, double-blind, controlled clinical trial. The results obtained showed that the erythema surface was significantly reduced, but did not differ significantly between the two groups (P> 0.05), as did the reduction of colony forming unit during treatment between the groups (P> 0.05) . The Candida type could not be correlated to the achieved clinical stage. The minimum inhibitory concentration test results showed that a concentration of 0,05 µg/ml chlorhexidine is already able to inhibit the growth of 85% of the tested strains. The primary hypothesis was confirmed, which can be interpreted as an accelerated clinical remission in both proposed therapies. Although secondary hypotheses have not been confirmed, Candida's persistence may be just colonization, and the morphological plasticity of Candida seems to be strongly influenced by fungal virulence. Thus, the results of the minimum inhibitory concentration analysis suggest that a subinhibitory concentration of chlorhexidine may be acting on the virulence of Candida spp. However, no results were found to confirm the clinical efficacy of 1% chlorhexidine diacetate incorporated into the soft liner in denture stomatitis.

Fundamento: la estomatitis subprótesis es una de las alteraciones que con más frecuencia se diagnostica dentro de la patología bucal. Objetivo: Caracterizar pacientes portadores de estomatitis subprótesis.Métodos: estudio descriptivo transversal sobre una muestra de 37 pacientes pertenecientes a la Clínica Estomatológica del Policlínico Universitario José Luis Chaviano Chávez, Área I, de Cienfuegos atendidos entre septiembre y diciembre de 2016. Se analizaron: sexo, edad, tipo de prótesis, hábito de uso, estado técnico de la prótesis, hábitos perjudiciales, grado y zona de la lesión.Resultados: predominó el sexo femenino, las edades entre 60 y 74 años. El tipo de prótesis más común fue la total, prevaleció como hábito perjudicial la higiene bucal deficiente; la mayor cantidad usaba continuamente las prótesis, el grado de mayor prevalencia resultó el II y la lesión se encontró con más frecuencia en la zona media; el mayor porcentaje de pacientes portaba prótesis desajustadas. Conclusiones: en los pacientes portadores de prótesis estomatológicas, la estomatitis subprótesis constituye indudablemente una lesión de etiología multifactorial.


Foundation: sub-prothesis stomatitis is one of the most frequently diagnosed disturbances within oral pathology. Objective: To characterize patients carrying subprothesis stomatitis. Methods: A descriptive cross-sectional study was carried out on a sample of 37 patients belonging to the José Luis Chaviano Chavez University Polyclinic Clinic, Area I, from Cienfuegos, attended between September and December 2016. Sex, age, type of prosthesis, use, technical state of the prosthesis, harmful habits, degree and area of injury were analyzed. Results: female gender predominated, between the ages of 60 and 74 years. The most common type of prosthesis was the total one, poor oral hygiene prevailed as a harmful habit, most the patients used prostheses continuously, the II degree was of higher prevalence and the lesion was more frequently found in the middle zone; the highest percentage of patients had maladjusted dentures. Conclusions: in patients with stomatologic prosthetics, sub-prothesis stomatitis constitutes undoubtedly a lesion with multifactorial etiology.

Fundamento: la estomatitis subprótesis es una enfermedad inflamatoria que afecta la mucosa oral que se encuentra en contacto con las prótesis dentales. Objetivo: establecer la efectividad terapéutica del oleozón y la crema de aloe al 25 % en el tratamiento de la estomatitis subprótesis. Métodos: se realizó un estudio experimental de evaluación de alternativas terapéuticas, en un universo de 96 pacientes con diagnóstico de estomatitis subprótesis tipo I y II en la Clínica Estomatológica Municipal Manuel Sánchez, Manzanillo, Granma, en el primer semestre del año 2016. Resultados: la estomatitis subprótesis tipo II fue identificada en los pacientes de ambos grupos de tratamiento. Solo se registró un paciente con una reacción adversa leve, consistente con ardor a la aplicación del oleozón. A los 10 días de seguimiento el oleozón resultó en un 100 % de efectividad terapéutica en los tipos IA y IB de estomatitis subprótesis mientras que en el tipo II fue un poco más baja. En este mismo periodo de tiempo la efectividad de la crema de aloe al 25 % fue efectiva en los tipos IA y IB mientras que en el tipo II esta efectividad fue de apenas de un porcentaje más bajo. Conclusiones: la estomatitis subprótesis es más prevalente en el sexo femenino donde el oleozón es una alternativa terapéutica efectiva y con resultados terapéuticos muy superiores, al mismo tiempo, el uso de crema de aloe al 25 %.


Background: the denture stomatitis is an inflammatory pathology that affects the mucous oral that is contact with the dental prosthesis. Objective: to establish the therapeutic effectiveness of the oleozón and the aloe cream to 25 % in the treatment of the denture stomatitis. Methods: a experimental study of evaluation of therapeutic alternatives in an universe of 96 patients with diagnostic of denture stomatitis type I and II in the Manuel Sánchez Municipal Stomatolgical Clinic, Manzanillo, Granma, in the first semester of the year 2016. Results: the denture stomatitis type II were identified in 39,58 % of the patients in both treatment groups. Alone a patient registered with a consistent light adverse reaction with ardor to the application of the oleozón. To the 10 days of pursuit the oleozón was in 100 % of effectiveness therapeutic in the types IA and IB of denture stomatitis while in the type II this it was of 84,21 %. In this same period of time the effectiveness of the aloe cream to 25 % was 94,12 % and 91,67 % in the types IA and IB respectively while in the type II this effectiveness was of hardly 68,42 %. Conclusions: the denture stomatitis continues being more prevalent in the female sex being the oleozón a highly effective therapeutic alternative and with very superior therapeutic results, in the time, to the use of aloe cream to 25 %.

Fundamento: La estomatitis subprótesis es una de las afecciones más frecuentes en pacientes portadores de prótesis dental y constituye un factor de riesgo para la aparición de lesiones premalignas y malignas de la cavidad bucal.Objetivo: describir el comportamiento de la estomatitis subprótesis en pacientes con prótesis dental superior.Métodos: se realizó un estudio descriptivo, de todos los pacientes portadores de prótesis dental superior (N=61) que acudieron a la consulta a causa de estomatitis subprótesis, en el periodo de septiembre 2014 a septiembre 2015. Se analizaron las variables: grupos de edades, sexo, grado de la lesión, estado de la prótesis, hábitos nocivos, ubicación de la lesión y frecuencia de las visitas al estomatólogo.Resultados: predominó el grupo de adultos mayores (54,1 %), y el grado II de la estomatitis subprótesis, representado por el 77 %. Se observaron prótesis desajustadas en la mayoría de los pacientes, y las lesiones se localizaron con más frecuencia en zona mixta. Los hábitos nocivos más frecuentes fueron el uso continuo y la higiene bucal deficiente. El 85,2 % de los pacientes plantearon que visitaron al estomatólogo solo cuando sintieron molestias.Conclusión: algunos resultados denotan el desconocimiento de los pacientes acerca de este padecimiento, entre ellos la alta frecuencia de hábitos nocivos como el uso continuo de la prótesis y la higiene bucal deficiente, factores que condicionan la aparición de estomatitis subprótesis.


Foundation: Sub prosthesis stomatitis is one of the most frequent affections in patients who wear dental prosthesis and constitutes a risk factor for the appearance of pre malignant and malignant lesions in the oral cavity.Objective: To describe the behaviour of sub prosthesis stomatitis in patients with upper dental prosthesis.Methods: A descriptive study was carried out, of all the patients who wear upper dental prosthesis (N=61) who came to the consultation due to sub prosthesis stomatitis, in the period from September 2014 to September 2015. The variables analyzed were: Age groups, sex, degree of the lesion, state of the prosthesis, toxic habits, location of the lesion and frequency of visits to the dentist.Results: Old adults predominated (54.1%) and sub prosthesis Stomatitis degree II, represented by 77%. Unadjusted prosthesis was observed in most patients and the lesions were located more frequently in mix zones. The most frequent toxic habits were continuous use and deficient oral hygiene. 85.2% of the patients reported to have visited the dentist only when they felt discomforts.Conclusion: Some results show patients ignorance about this ailment, among them a high frequency of toxic habits so as the use of continuous use of prosthesis and deficient oral hygiene, factors which condition the appearance of sub prosthesis stomatitis.

A estomatite por prótese (EP) é uma condição multifatorial que acomete frequentemente usuários de prótese total e geralmente é relacionada com Candida albicans. Devido aos efeitos tóxicos da terapia antifúngica, novas terapias para EP são necessárias. Objetivos: O objetivo deste estudo foi avaliar a eficácia do extrato aquoso de Buchenavia tomentosa e bicarbonato de sódio frente a C. albicans em um modelo de EP em ratos. Material e Métodos: Um aparelho de resina acrílica simulando a base da prótese total foi fixado cobrindo o palato de 48 ratos machos seguido por indução da candidose. Os ratos foram divididos em 4 grupos (n=12): controle, bicarbonato de sódio, B. tomentosa e nistatina (controle positivo). Cada grupo foi subdividido de acordo com o período de tratamento; 24 horas (n=6) e 48 horas (n=6). Os animais foram sacrificados e os aparelhos foram removidos para contagem de C. albicans e análise por microscopia eletrônica de varredura. Resultados: Após 24 horas de tratamento, observou-se redução significativa da contagem de C. albicans tanto B. tomentosa quanto nistatina (nistatina x controle, p<0,01; B. tomentosa x controle, p=0,03). Os resultados foram confirmados pela análise histológica. Conclusão: Tanto o extrato aquoso de B. tomentosa e o bicarbonato de sódio foram capazes de reduzir significativamente as contagens de C. albicans em modelo experimental de EP (AU)


Background: Denture stomatitis (DS) is a multifactorial condition that commonly affects denture users and is mainly caused by Candida albicans. Due to the toxic effects of antifungal therapy, new therapies for DS are claimed. Objective: The aim of the study was to evaluate the efficacy of aqueous extract of Buchenavia tomentosa and sodium bicarbonate against C. albicans in a model of DS in rats. Material and Methods: An acrylic resin device simulating a denture base was fixed covering the palate of forty-eight male rats followed by candidiasis induction. Rats were divided into 4 groups (n = 12): Control, sodium bicarbonate, B. tomentosa and nystatin (positive control). Each group was subdivided according to the period of treatment; 24 h (n = 6) and 48 h (n = 6). Animals were sacrificed and had their devices removed for C. albicans counts and SEM analysis. The palate mucosa was removed and processed for histopathologic analysis. Results: After 24 h of treatment, both B. tomentosa and nystatin groups reduced significantly C. albicans counts when compared to control (nystatin x control, p < 0.01; B. tomentosa x control, p = 0.03). The results were confirmed by the histologic analysis. Conclusion: Both the aqueous extract of B. tomentosa and sodium bicarbonate was able to significantly decrease C. albicans counts in an experimental model of DS (AU)

Fundamento: la solicitud de reparaciones protésicas no constituye una urgencia en estomatología, pero ello origina una oportunidad de revisión de la cavidad bucal. Objetivo: establecer los aspectos de la salud bucal y el estado prostodóncico de los pacientes solicitantes de algún tipo de reparación protésica. Métodos: se realizó un estudio cuantitativo, longitudinal y descriptivo, de series de casos en 482 pacientes que solicitaron algún tipo de reparación protésica en consulta especializada de la Clínica Estomatológica Municipal “Manuel Sánchez”, Manzanillo, Granma, en el primer semestre del año 2016. Resultados: el mayor por ciento de los pacientes, que solicitaron algún tipo de reparación protésica, eran del sexo femenino. La media de edad se encontró en un rango de 28-86 años. La solicitud de reparaciones protésicas fue mayor en prótesis totales y superiores, fueron las fracturas completas el principal motivo de solicitud. Las revisiones de la cavidad bucal fueron en disminución con el tiempo de uso de las prótesis. El factor de riesgo más referido fue la frecuencia y métodos incorrectos de higiene bucal mientras que la estomatitis subprótesis fue la lesión bucal más constatada. Conclusiones: las solicitudes de reparación protésica se deben, en su mayoría, a fracturas completas y estas son hechas por pacientes que tienen una frecuencia y métodos incorrectos de higiene bucal, lo que determinó que la estomatitis subprótesis fuera la lesión más encontrada.


Background: the requesting denture repairs doesn't constitute an urgency in stomatology but this originates an opportunity of revision of the buccal cavity. Objective: to establish the health buccal aspects and the prosthodontics state of the patient applicants of some type of denture repairs. Methods: a quantitative, longitudinal and descriptive study of cases series in 482 patients requesting some type of denture repairs in specialized consults, in the Manuel Sánchez Municipal Stomatolgical Clinic, Manzanillo, Granma, in the first semester of the year 2016. Results: the 56,85 % of the patients that requested some type of denture repairs, was of the female sex. The age stocking was of 63,27 years in a 28-86 year-old range. The application of denture repairs was bigger in total denture (55,81 %) superiors (53,94 %) being the complete fractures (38,59 %) the main application reason. The revisions of the buccal cavity were diminishing with the time of use of the denture. The risk factor of more referred was the frequency and incorrect methods of buccal hygiene (83,2 %) while the denture stomatitis was the verified buccal lesion (21,16 %). Conclusions: the applications of denture repairs owe you, in their majority, to complete fractures and these they are made by patients that have a frequency and incorrect methods of buccal hygiene what determined that the denture stomatitis was the most opposing lesion.

A presente pesquisa de campo tem o objetivo de verificar a prevalência das lesões bucais cancerizáveis diagnosticadas nas campanhas do Projeto Asa Branca que foram realizadas pela FOC/ASCES no estado de Pernambuco, segundo gênero e idade nos anos de 2004 a 2008. Iniciou-se uma análise de todas as fichas cadastrais arquivadas das campanhas realizadas nesses anos. Os dados encontrados foram separados de acordo com as variáveis sexo e faixa etária, e, logo após, utilizou-se o teste estatístico qui-quadrado. Os resultados mostraram que nos anos de 2004 a 2008, 19.397 pessoas apresentaram algum tipo de lesão bucal, sendo que as lesões predominaram no sexo feminino (13.826 lesionadas - 71,28%) e nas pessoas de 30 a 59 anos de idade (10.266 lesionados - 52,93%). Foi concluído que as lesões prevalecem no sexo feminino e nas faixas etárias entre 30 a 59 anos. Como o índice de portadores de próteses dentárias é superior no sexo feminino com essa faixa etária, sugere-se, portanto, que a maioria dos casos das lesões na população pernambucana é devido a próteses dentárias mal adaptadas ou a má higienização da prótese, causando a lesão mais diagnosticada nessas campanhas: a estomatite protética.


This research field has the purpose to verify the prevalence of carcinogenic oral injuries diagnosed in campaings of Project Asa Branca wich were realized by FOC/ASCES in state of Pernambuco, according to gender and age, in the years 2004-2008. An analysis of all the registration records filed of Project Asa Branca campaings in these years has began. The informations found were separate according to variables gender and age, and after, was used the chi-square statistical test. The results showed in years 2004 to 2008, 19.397 people showed some type of oral lesion, being the lesions predominated in female gender (13.826 injureds - 71,28%) and in people with 30 to 59 years old (10.266 injureds - 52,93%). It was verified that, the lesions predominated in female gender and in age group 30 to 59 years old. As the index of patients with prosthesis is more in female gender with these age, we suggest, so, the majority of lesions cases in the Pernambuco state population is because dental prosthesis with bad adaptation or the bad hygiene of the prosthesis, causing the lesion more diagnosed in these campaings: the stomatitis denture.

Estomatite protética associada a Cândida (EPC), a lesão mais frequente em usuários de próteses removíveis, principalmente os idosos, caracteriza-se por uma inflamação da mucosa bucal que suporta a prótese. Está fortemente associada com Candida Albicans, bem como com fatores locais e sistêmicos, como a deficiência da resposta imune. Os monócitos são importantes na resposta protetora contra o fungo, produzindo citocinas que recrutam e ativam leucócitos. Existem alterações funcionais dessas células com o avanço da idade. Não foi possível encontrar na literatura dados referentes à função imunomodulatória dos monócitos de idosos com EPC. O presente trabalho pretendeu avaliar a produção de citocinas por essas células, estimuladas in vitro com C. albicans, obtidas do sangue periférico de idosos usuários de prótese total superior (PTS) com EPC, comparando-se com idosos usuários de PTS sem EPC, e com idosos e jovens não usuários de PTS. Os monócitos isolados foram cultivados em placas de cultura de 24 poços, na ausência ou presença de lipopolissacarídeo (LPS) ou C. albicans ATCC 90028 morta pelo calor. Após 18 horas, o sobrenadante foi coletado e submetido ao ensaio de imunoabsorção por ligação enzimática (ELISA) para dosagem das citocinas pró- inflamatórias fator de necrose tumoral- (TNF-), interleucina-6 (IL-6), IL-1, CXCL8 e proteína quimiotática de monócito (MCP-1), e anti-inflamatórias IL-10 e fator transformador de crescimento- (TGF-). Os resultados estão expressos como média ± desvio padrão dos valores obtidos para cada grupo, e foram analisados por meio de testes estatísticos não-paramétricos. Valores de p<0,05 foram considerados estatisticamente significativos. Os resultados demonstraram, de uma forma geral, alterações nos monócitos oriundos dos idosos com EPC, em comparação aos outros grupos: menor produção espontânea de CXCL8 e de MCP-1; menores níveis de TNF-, de IL-6, de IL-1, de CXCL8, de MCP-1 e de IL-10, após estímulo com LPS; e menor...


Candida-associated denture stomatitis (DS), the most frequent lesion among denture wearers, especially the elderly, is characterized by inflammation of the denture-bearing mucosa. It is strongly associated with Candida albicans, as well as with local and systemic factors, such as impaired immune response. Monocytes are important in the protective immune response against the fungus, by the production of cytokines that recruit and activate leukocytes. There are functional changes of these cells with advancing age. No data were found in the literature concerning the immunomodulatory function of these phagocytes in elderly patients with DS. This study aimed to evaluate the cytokine production by monocytes, challenged in vitro with C. albicans, obtained from peripheral blood of elderly denture wearers with DS, compared with elderly denture wearers without DS and elderly and young non-denture wearers. The isolated monocytes were cultivated in 24-well flat-bottomed culture plates, in the absence or presence of lipopolysaccharide (LPS) or heat-killed C.albicans ATCC 90028. After 18 hours, the supernatant was collected and submitted to the enzyme-linked immunosorbent assay (ELISA) for determination of the pro-inflammatory cytokines tumor necrosis factor-alpha (TNF-), interleukin-6 (IL-6), CXCL8, IL-1, monocyte chemotactic protein-1 (MCP-1) and anti-inflammatory cytokines IL-10 and transforming growth factor- (TGF-). The results are expressed as mean ± standard deviation of the values obtained for each group, and were analyzed using nonparametric statistical tests; p values <0.05 were considered statistically significant. The results demonstrated, in general, changes in monocytes from the elderly with DS, as compared to other groups: lower spontaneous production of CXCL8 and MCP-1; lower levels of TNF-, IL-6, IL-1, CXCL8, MCP-1 and IL-10 after stimulation with LPS; and reduced production of TNF- and IL-6 after stimulation with C. albicans. ABSTRACT

A Candida albicans tem sido considerada o principal agente etiológico da estomatite protética, uma das infecções mais comumente observadas em pacientes usuários de próteses removíveis. A adesão de C. albicans à superfície das próteses é o primeiro passo para o desenvolvimento da estomatite protética. Assim, uma adequada higienização das próteses é essencial para prevenir a formação de biofilme microbiano sobre a superfície protética e, conseqüentemente, o início e propagação desta infecção. O objetivo deste estudo foi avaliar a efetividade da escovação com diferentes soluções (água destilada, dentifrício, digluconato de clorexidina a 2%, hipoclorito de sódio a 1% e Polident fresh cleanse®) na redução da viabilidade de um biofilme maduro de C. albicans, desenvolvido sobre uma resina termopolimerizável para base de prótese. Para isso, 90 corpos-de-prova circulares foram confeccionados, esterilizados e inoculados com uma suspensão de 107 células/mL de C. albicans. Para a formação do biofilme, todos os corpos-de-prova foram incubados por 48 h a 37 °C sob agitação. A seguir, os corpos-de-prova foram aleatoriamente divididos (n=9) e individualmente submetidos à escovação ou exposição por 90 s nas diferentes soluções avaliadas. Os espécimes imersos em água destilada pelo mesmo período de tempo foram utilizados como controle positivo. Para verificar a efetividade da escovação e dos agentes de limpeza, os corpos-de-prova foram submetidos à avaliação da atividade metabólica das células não removidas de C. albicans por meio do teste do XTT. Os resultados obtidos foram analisados estatisticamente pelos testes de Mann-Whitney e Kruskal-Wallis (α=0.05). A escovação com todos os agentes de limpeza apresentou redução significativamente superior (p<0,0001) na viabilidade do biofilme quando comparada à exposição dos corpos-de-prova às soluções. Escovação e exposição em digluconato de clorexidina a 2% e escovação com hipoclorito de sódio a 1% resultaram em 100% de inativação do biofilme de C. albicans. Escovação e exposição nas demais soluções demonstraram, ao menos, 88% de redução na viabilidade celular (p<0,0001). O método de escovação associado ao uso dos agentes de limpeza digluconato de clorexidina a 2% e hipoclorito de sódio a 1% provou ser efetivo para inativar biofilme maduro de C. albicans aderido em resinas acrílicas para base de prótese


The adhesion of C. albicans to surfaces is the prerequisite for occurrence of denture stomatitis, a common disease diagnosed among denture wearers. Therefore, a strict routine of denture cleansing is essential to prevent biofilm formation on the acrylic denture surface and the onset of this infection. Thus, the aim of the current study was to investigate the effectiveness of combining toothbrushing and cleansing agents (dentifrice, 2% chlorhexidine gluconate, 1% sodium hypochlorite, and Polident fresh cleanse®) in inactivating C. albicans biofilm. Circular specimens (10 x 2 mm) of acrylic resin denture base material were made, sterilized, and individually inoculated with C. albicans (1x107 colony-forming units/mL). After incubation (37 °C/48 h), the specimens were divided into 10 experimental groups (n=9): 5 subjected to toothbrushing with different cleansing agents and 4 exposed to the cleansing agents without toothbrushing. Non–cleansed specimens were used as positive controls. The viability of cells was evaluated by XTT reduction method. Mann-Whitney and Kruskal-Wallis tests were used for comparative analysis between the two methods of cleansing and the cleansing agents, respectively (α=0.05). Toothbrushing with all agents was significantly more effective (p<0.0001) in reducing biofilm viability than exposure to the cleansing agents only. Exposure to all cleansing agents reduced significantly (p<0.0001) the biofilm viability, with 2% chlorhexidine gluconate being most effective (p<0.0001). Toothbrushing with 2% chlorhexidine gluconate and 1% sodium hypochlorite resulted in 100% inactivation of the biofilm. The use of cleansing agents as an adjunct to toothbrushing is an effective method to reduce C. albicans biofilm viability on denture base resins.

ABSTRACT

Este estudo in vivo teve como objetivos avaliar: 1. a prevalência das diferentes espécies de Candida em pacientes diabéticos portadores de estomatite protética e compará-la àquela de pacientes não diabéticos com/sem estomatite protética; 2. a eficiência de dois tratamentos para estomatite protética em pacientes diabéticos: terapia antifúngica com nistatina e desinfecção de próteses totais por micro-ondas. Para isso, 210 pacientes usuários de prótese totais foram selecionados e distribuídos em três grupos de estudo. O grupo controle - GC foi formado por 90 pacientes não diabéticos e com mucosa oral saudável; os grupos experimentais foram formados por portadores de estomatite protética, sendo 80 pacientes não diabéticos - EPND e 40 diabéticos do tipo 2 controlados - EPD. Uma avaliação intra-oral dos pacientes dos grupos experimentais foi realizada e a aparência clínica da mucosa inflamada foi classificada de acordo com os critérios de Newton em graus I, II ou III. Coletas microbiológicas das próteses totais foram realizadas com swab estéril e plaqueadas em meio CHROMagar Candida. Em seguida, os procedimentos de identificação foram realizados: análise de microcultivo, teste de triagem fenotípica em caldo hipertônico e testes bioquímicos (ID-32C). Os resultados obtidos foram analisados por Intervalo de Confiança de Bonferroni e testes Exato de Fisher e Qui Quadrado (α=0,05). Os 40 pacientes diabéticos foram, então, submetidos ao tratamento da estomatite protética. Para isso, foram aleatoriamente distribuídos em 2 grupos (n=20): grupo NIS ­ tratamento com antifúngico tópico nistatina (suspensão oral - 100.000UI/mL), 4 vezes ao dia, por 14 dias; grupo MIC ­ tratamento por meio da irradiação das totais por micro-ondas (650W / 3 minutos), 3 vezes por semana, por 14 dias. Dois parâmetros foram utilizados para avaliar a efetividade dos tratamentos: microbiológico (coletas de material das próteses e das mucosas palatinas com swab estéril para quantificação de colônias viáveis ­ ufc/mL e identificação de Candida spp.) e clínico (tomadas fotográficas intra-orais das mucosas palatinas infectadas). Todos os pacientes foram submetidos a estes procedimentos previamente ao início dos tratamentos, ao seu final, e 30, 60 e 90 dias decorridos do seu início. Os valores de log10ufc/mL foram analisados estatisticamente por ANOVA subjacente a um modelo linear com efeitos aleatórios. A prevalência das espécies de Candida foi avaliada pelo teste Qui Quadrado. As avaliações qualitativas das fotografias intra-bucais e a análise da inflamação ao longo do tempo foram avaliadas, respectivamente, pelos testes Exato de Fisher e Mann-Whitney (α>05). Os resultados observados demonstraram que a C. albicans foi a espécie predominante em todos os grupos de estudo (p<0,016), seguida por C. tropicalis e C. glabrata (p>0,05). Não foram verificadas diferenças significantes (p>0,05) na prevalência de Candida spp. entre os grupos EPND e EPD. As prevalências de C. albicans e C. tropicalis foram significativamente superiores (p<0,01) nos grupos de pacientes com estomatite protética e a de C. tropicalis foi superior (p<0,05) nos pacientes diagnosticados com infecção grau III de Newton. Com relação aos tratamentos instituídos nos pacientes diabéticos, ambos reduziram significativamente (p<0,001) os valores de log10ufc/mL após 15 e 30 dias, não tendo sido encontradas diferenças estatisticamente significantes (p>0,05) entre os grupos de estudo. Não foram verificadas diferenças significantes (p>0,05) entre o número de pacientes curados e com recidivas, após a condução dos dois tratamentos. Com base nestes resultados, foi concluído que a prevalência de Candida spp. foi similar entre os pacientes diabéticos e não-diabéticos com estomatite protética. Apesar de a C.albicans ter sido a espécie mais frequentemente identificada em todos os pacientes avaliados, a prevalência de C. tropicalis foi superior nos pacientes com estomatite protética e naqueles com maior grau de inflamação, sugerindo que esta espécie pode estar envolvida com o início e a progressão da infecção. Além disso, a irradiação de próteses totais por micro-ondas pode ser utilizada com sucesso para tratar e prevenir o aparecimento de estomatite protética em pacientes diabéticos, apresentando a mesma efetividade de um método clássico de tratamento com antifúngico tópico


The aim of this in vivo study was to evaluate: 1. the prevalence of Candida spp. in well-controlled type 2 diabetic patients and compare it to that found in non-diabetics with/without denture stomatitis; 2. the efficacy of two treatments of denture stomatitis in diabetic patients: topical nystatin and denture microwave disinfection. Two hundred and ten denture wearer patients were divided into three groups of study. The control group - CG was formed by 90 individuals without diabetes or denture stomatitis and the experimental groups were formed by patients with denture stomatitis, being 80 non-diabetics - DSND and 40 well-controlled type 2 diabetics - DSD. Mucosal characteristics of denture stomatitis patients were classified in types I, II, and III, according to Newton's criteria. Mycological samples were taken from the dentures and cultured in CHROMagar's plates. Candida spp. were identified by micro-cultivation, hypertonic Sabouraud broth, and bioMérieux ID32C assays. Results were analyzed by means of Bonferroni corrected confidence interval, Fisher's exact test, and Chi-square analysis of several proportions (α=0.05). The 40 diabetic patients were divided into two groups (n=20) and submitted to two treatments for denture stomatitis: NYS group ­ patients were treated with topical nystatin, 4 times a day, for 14 days; MW group ­ patients had their dentures microwaved (650W / 3 minutes), 3 times per week, for 14 days. Mycological samples were taken from the palates and the tissue surface of the dentures for quantification and identification of Candida spp. and standardized photographs of the palates were taken for the clinical analysis. Microbiological and clinical evaluations were repeated at baseline, the end of treatments (day-14), and follow up (day-30, day60, day-90). Microbiological data were evaluated by ANOVA using a random effects statistical model,Tukey's post hoc test, and Chi-square test, while clinical results were analyzed by Mann-Whitney and Fisher's exact tests (α=0.05). According to the results, C. albicans was the most predominant species isolated in CG, DSND, and DSD groups (p<0.016), followed by C. tropicalis and C. glabrata (p>0.05). No significant differences were found in Candida spp. prevalence between DSND and DSD (p>0.05). The percentage frequencies of C. albicans and C. tropicalis in denture stomatitis groups (DSND and DSD) were significantly higher (p<0.01) than those in CG and the prevalence of C. tropicalis was significantly higher (p<0.05) in Newton's type III patients. Both NYS and MW treatments were considered successful in reducing the clinical signs of denture stomatitis and significantly reduced the values of cfu/mL from the palates and dentures at day-14 and day-30. Forty percent of patients had no signs of denture stomatitis at the end of both treatments. No significant differences (p>0.05) in the microbiological and clinical outcomes were revealed between the two treatments. It was concluded that the prevalence of Candida spp. was similar between diabetic and non-diabetic patients with denture stomatitis. Despite C. albicans was the most commonly isolated organism, the prevalence of C. tropicalis increased in patients with denture stomatitis and in those with the highest degree of inflammation, suggesting that this species may play an important role in the progression of this infection. In addition, denture microwave disinfection was as effective as nystatin on the treatment of well-controlled type 2 diabetic patients with denture stomatitis

Objeti vo: Avaliar a efi cácia clínica da ti ntura da aroeira notratamento da estomati te protéti ca.Método: Foram selecionados 18 pacientes usuários de prótesesremovíveis com diagnósti co clínico para estomati te protéti cati po II e presença de candidose associada à prótese, constatadosa parti r de um exame clínico e micológico. Os pacientes foramdistribuídos em dois grupos: GT (grupo teste) - tratamentocom a ti ntura da aroeira; GC (grupo controle), tratamento comnistati na. Todos os pacientes foram orientados a higienizara prótese com escova e denti frício e, em seguida, aplicar oproduto na mucosa palati na e na superfí cie da prótese 3 vezesao dia, durante 15 dias consecuti vos, remover a prótese à noitee armazená-la em um recipiente com água. No 15º dia de uso,foi realizado um novo exame clínico e micológico para avaliara efi cácia do tratamento. Os dados foram analisados com ostestes Wilcoxon e Mann-Whitney com nível de signfi cância de5%.Resultados: Observou-se eliminação do processo infl amatórioe da infecção por Candida spp. em 66,7% e 77,8% dos casos,respecti vamente, para GT. Já para GC, a eliminação doprocesso infl amatório e da infecção fúngica ocorreu em 77,8%e 88,9% dos casos, respecti vamente. Estes resultados foramestati sti camente signifi cantes (Wilcoxon p=0,01). Não foiobservada diferença estatí sti camente signifi cante entre os doistratamentos (Mann-Whitney p>0,05). Objecti ve: To evaluate the clinical effi cacy of Schinusterebinthifolius Raddi (aroeira) ti ncture in the treatment ofdenture stomati ti s.Method: Eighteen removable denture wearers with clinicaldiagnosis of type II denture stomati ti s and presence ofcandidosis associated to the denture use, as confi rmed byclinical and mycological examinati ons, were selected forthe study. The pati ents were allocated to two groups: TG(test group) - treatment with Schinus terebinthifolius Raddi(aroeira) ti ncture; CG (control group), treatment with nystati n.All pati ents were instructed to clean the dentures withtoothbrush and denti frice, and then apply the product on thepalatal mucosa and on denture surface 3 ti mes a day, during15 consecuti ve days, removing the denture at bedti me andkeeping it in a receptacle with water. At the 15th day of use, theclinical and mycological examinati ons were redone to evaluatetreatment effi cacy. Data were analyzed stati sti cally by Wilcoxonand Mann-Whitney tests at 5% signifi cance level.Results: The infl ammatory process and Candida spp. infecti onwere eliminated in 66.7% and 77.8% of the cases, respecti vely,in TG. In CG, eliminati on of the infl ammatory process andfungal infecti on occurred in 77.8% and 88.9% of the cases,respecti vely. ABSTRACT

Fundamento: Cuando la candidiasis se asocia con la prótesis removible puede inducir a la formación de estomatitis subprótesis, esencialmente en aparatos desajustados y con muchos años de uso. Objetivo: Determinar la prevalencia de la candidiasis en pacientes con estomatitis subprótesis. Método: Se realizó un estudio observacional, descriptivo y transversal en la Clínica Estomatológica Docente de La Vigía desde enero a marzo de 2008 en 30 pacientes mayores de 20 años y de ambos sexos con estomatitis subprótesis (10 en cada grado clínico de la lesión), portadores de prótesis removibles. Resultados: El comportamiento microbiológico de los pacientes con estomatitis subprótesis afloró que no todos los pacientes afectados presentaron candidiasis, solo se evidenció la presencia de la misma en 21 para un 70 por ciento de la muestra. Según el grado de la lesión esta afección se presentó en cuatro pacientes de grado I para un 19,04 por ciento, 7 del grado II para un 33,33 por ciento y en el 100 por ciento del grado III con diferencia significativa entre la proporción de pacientes con cándida de los grados I y II con respecto al grado III. En el estudio predominaron los pacientes con afecciones de grado III. Conclusiones: No todos los pacientes con estomatitis subprótesis presentaron cándida albicans. Se incrementó la candidiasis en los grados avanzados de la estomatitis subprótesis. La totalidad de las prótesis en pacientes con lesiones grado II y III presentaron cándida albicans.


Background: When candidiasis is associated with removable prosthesis may induced to the formation of stomatitis sub-prothesis, essentially in non-adjusted appliance and with many years of use. Objective: To determine the prevalence of candidiasis in patients with stomatitis sub-prosthesis. Method: An observational, descriptive and cross-sectional study at La Vigía Teaching Odontology Clinic from January to March 2008 was conducted in 30 patients older than 20 years and from both sexes with stomatitis sub-prosthesis (10 in each clinical degree of the lesion), carriers of removable prosthesis. Results: The microbiological behavior of patients with stomatitis sub-prosthesis showed that not all the affected patients presented candidiasis, only showed its presence in 21 for a 70 percent of the sample. According to the degree of the lesion this affection was presented in four patients of I degree for a 19,04 percent, 7 of II degree for a 33,33 percent and in the 100 percent of III degree with significant difference among the proportion of patients with Candida of I and II degrees regarding to III degree. In the study predominated patients with affections of III degree. Conclusions: Not all the patients with stomatitis sub-prosthesis showed Candida albicans. Candidiasis in advanced degrees of the stomatitis sub-prothesis was incremented. The totality of the prosthesis in patients with II and III degree lesions showed Candida albicans.
